A healthcare professional visits the family at home to allow follow-up and assessment of health status while expediting referrals to their treating physicians in case any red flag is identified during medical examination. Meanwhile the follow-up visits also include checking up on compliance and adherence to their doctors’ instructions for lifestyle modifications and prescribed medications. Parents are more aware of their condition and closely controlled for better outcomes.
